I have lots of unused physicians, and not very good medical care. Clearly the last player did a good job of training physicians to improve the medical care and I realise should invest in the physicians to improve the medical care, but how and how much should I invest?

Just allocate the physicians to work in the community.
No need to invest anything, they run private practices and charge their patients accordingly.

to provide a poor medical service, allocate 1 physician per10,000 population.
To provide a reasonable medical service, allocate 1 physician per 1000 population.
to provide a good medical service, allocate 1 physician per 100 population.
no monies required.

If you have them, best to use them. Like others have said it doesn't require funding. Though I can't prove it, I think better medical care does benefit honour

no trained physicians, fairly random outbreaks of disease.
poor medical service, cover is patchy, but the chances of an outbreak spreading are reduced.
reasonable medical service, a decent medical system, reducing the risk of outbreaks happening in the first place.
good medical service, superb cover, outbreaks are less likely and population growth likely to be at a higher level.
